Social Media Strategist
The TVCCA Client Services Case Manager is a ten month temporary position commencing upon hire. This position will be working with the TVCCA Community Resource Coordinator, position responsibilities include calling residents of New London or Windham Counties, identified by the Local Health Departments in Region 4 and or the State of Connecticut Health department as needing to quarantine or self- isolate due to confirmation of, or possible exposure to Covid-19. The calls are placed to individuals & families who have identified an immediate basic need to quarantine and or isolate for 10- 14 days during the contact tracing interview conducted by Local Health Departments in Region 4 and or State of Connecticut Contact screening. The client services case manager will work with the residents identifying the immediate basic needs, discuss available client resources, supports and access. When immediate service needs not available through local resources the Case Manager will work with the Community Resources Coordinator to identify other means of availability and or solutions. The Client Services Case Manager shall need to have a robust knowledge of local resources and services. This position will require travel predominately New London County with possibility of Windham to coordinate and deliver required needs to clients. NOTE: (There is no direct client visit required, but rather front door drop offs) Duties include calling clients identified in the ContaCT system software, follow up calls to check on status, resource referral, arranging, scheduling, working to delivery and or secure the resources to meet the immediate basic needs for the short term emergency case management. Preparation of daily, weekly & monthly reports.
Work with other TVCCA departments and staff for other services provided by TVCCA that potentially may and or will assist the individuals and families after the Quarantine and or Isolation time frame.
TVCCA philosophy works with clients holistically to meet and identify immediate needs but to also identify services for the future to aid the individual and family.
This position is Sunday through Thursday 9-5. Requires strong proficient computer skills in Microsoft office, good communication skills, empathy for the immediate health situation the clients and or families are facing
This a temporary 10month position.
Required Skills
Bachelor’s Degree plus two years of related work experience, or Associates Degree plus four years of related work experience.
· Experience providing strength-based case management services to singles and families.
· Demonstrate a history of working with minimal supervision in a mature manner.
· Excellent computer skills (Microsoft Office Suite, Excel, Access and Publisher).
· Strong communication skills, bi-lingual preferred.
· Possess a valid driver’s license and daily access to a reliable and insured motor vehicle
