Care Coordinator Blue Cross and Blue Shield of Alabama 41 reviews - Birmingham, AL Primary Responsibilities This position is responsible for developing, coordinating, and administrating activities relating to individual case management for Blue Cross Blue Shield of Alabama members. This position requires communicating with members of the health care delivery system, subscribers and their family members, group representatives, utilization review, and marketing. Activities include individual patient assessment, performance of case management activities, monitoring group specific data for intervention and reporting, performance of presentations for groups, participation in special projects as requested by management, and participation in and continuous review of NCQA standards.
