Patient Representative (TPA, Transition Patient Advocate) - Charleston, SC - Overview Summary Duties The Transition Patient Advocate (TPA) Patient Representative assists patients, their families and representatives, and facility staff members in recognizing and removing institutional barriers to the provision of optimum health care to Veterans, Identifies existing and potential problem areas, suggest solutions or alternatives to existing procedures which contribute to these problems. The incumbent will participate and assist in planning and executing outreach events as assigned by the TCM Program Manager. This may include but is not limited to military sponsored events, Veteran and community events, and VA sponsored events. These events are designed to provide education about VA healthcare and benefits and the opportunity to enroll for VA health care. Some TPA's will have the ability to register Veterans for VA healthcare, creating expedited access. The TPA acts to resolve problems, expedites services, or implement necessary corrective measures within established facility policies and where appropriate. May have the authority to review files or records and discuss with facility staff practices that appears to violate patient's rights or which causes unnecessary discomfort to patients, their families or health care facility. The incumbent will report such findings with recommendations for improvements to TCM Program Manager. May have shared responsibility for the resolution of patient problems with arise. Acts as an advocate for patient problems which arise. Acts as an advocate for patients, using seasoned judgment to resolve problems. The incumbent will collaborate with the TCM Program Manager and may cross systems as appropriate in resolution of problems in matters concerning patient care.
