    YOUR ROLE AT PFH:

    As the Team Leader, you are responsible for establishing, administering and directing the Assertive Community Treatment team as they provide needed treatment, rehabilitation, and support services to identified clients with severe and persistent mental illness and/or co-occurring disorders. The team leader will supervise and evaluate the multidisciplinary team in conjunction with coordinating with an appropriate psychiatric care provider to ensure effective implementation of the ACT model.

     You will:

    • Under the Program Director's guidance, will work to establish effective working relationships with other agencies in the communities served. Participate in selection of off-site sources for needed services not provided by the program, informing other agencies about the services offered by PFH.
    • Complete monthly file review for the program, ensuring that standards are being maintained as well as monitoring compliance with program and agency Quality Improvement Indicators. Follow up with any deficiencies during weekly staffing meetings and assist staff with correction of deficiencies.
    • Facilitate daily staff meetings and treatment planning meetings; direct daily clinical operations of the ACT Team including scheduling staff work hours to assure coverage for all shifts including on call services; continuously evaluate the status of clients and do appropriate planning and coordination of treatment activities to ensure immediate attention to changing needs.
    • Maintain required weekly contact hours: including directing and coordinating the admission process and treatment, rehabilitation and support services of the program in coordination with the psychiatric provider. Schedule intake assessments; develop and coordinate initial treatment plan, and provide clinical supervision of the development of the comprehensive assessments and treatment plans for each client. Provide direct service in the form of individual therapy and crisis management in compliance with recommended number of direct service hours per the TMACT ACT fidelity protocol.
    • Complete and review written reports to referral sources as needed in a timely manner. Promote integration of clinical and community support services and works as a team leader.

    WE THINK YOU'D BE GREAT FOR THIS ROLE IF YOU ARE:

    1. A physician licensed under Missouri law to practice medicine or osteopathy and with training in mental health services or 1 year of experience, under supervision, in treating problems related to mental illness or specialized training;
    2. A psychiatrist, a physician licensed under Missouri law who has successfully completed a training program in psychiatry approved by the American Medical Association, the American Osteopathic Association or other training program identified as equivalent by the department;
    3. A psychologist licensed under Missouri law to practice psychology with specialized training in mental health services;
    4. A professional counselor licensed under Missouri law to practice counseling and with specialized training in mental health services;
    5. A clinical social worker licensed under Missouri law with a master's degree in social work from an accredited program and with specialized training in mental health services;
    6. A psychiatric nurse, a registered professional nurse licensed under Chapter 335, RSMo with at least 2 years of experience in a psychiatric setting or a master's degree in psychiatric nursing;
    7. An individual possessing a master's or doctorate degree in counseling and guidance, rehabilitation counseling and guidance, rehabilitation counseling, vocational counseling, psychology, pastoral counseling or family therapy or related field who has successfully completed a practicum or has 1 year of experience under the supervision of a mental health professional;
    8. An occupational therapist certified by the American Occupational Therapy Certification Board, registered in Missouri, has a bachelor's degree and has completed a practicum in a psychiatric setting or has 1 of experience in a psychiatric setting, or has a master's degree and has completed either a practicum in a psychiatric setting or has 1 year of experience in a psychiatric setting;
    9. An advanced practice nurse—as set forth in section 335.011, RSMo, a nurse who has had education beyond the basic nursing education and is certified by a nationally recognized professional organization as having a nursing specialty, or who meets criteria for advanced practice nurses established by the Board of Nursing; and
    10. A psychiatric pharmacist as defined in 9 CSR 30-4.030; (any of above) with 1 year of supervisory experience and a minimum of 2 years' experience working with adults with serious mental illness in community settings, and is full time.

    Per TMACT Protocol – 1.0 FTE, master's degree in social work, psychology, psychiatric rehabilitation or related field, at least 3 years of experience working with individuals with severe mental illness. Is fully licensed as a counselor or social worker; PLPC will not fulfill fidelity requirements. Does not have commitments outside of the ACT program nor is role split
