Kelaca is looking for a Care Navigator for our client partner, The Family Heart Foundation in Chapel Hill, NC (Hybrid). We are actively seeking a qualified individual to work as a member of their Care Navigation Center team, providing information, resources, and support to individuals and families navigating diagnosis, care, and family screening. The ideal candidate would be experienced working with individuals in a healthcare setting, knowledgeable or willing to learn about hereditary lipid disorders, and have a passion for supporting individuals facing challenges related to their health. Critical to this role is the ability to speak fluent English and Spanish.
The Family Heart Foundation is a national, nonprofit research and advocacy organization. Their organization is dedicated to improving the diagnosis and care of inherited lipid disorders, familial hypercholesterolemia (FH) and elevated Lipoprotein(a) [Lp(a)], through ongoing education, support, and direct services to individuals, families and caregivers. In addition, the Family Heart Foundation programs address barriers to improving the diagnosis and care of inherited lipid disorders and consequent cardiovascular disease.
The Family Heart Foundation houses a Care Navigation Center devoted to helping individuals impacted by high cholesterol, cardiovascular disease, FH or elevated Lp(a) get the care they need, including finding the right specialist, understanding their diagnosis, treatment goals, and treatment options, navigating insurance, and cascade family screening.
Key Responsibilities:
The Care Navigator works as a member of the Care Navigation Center Team.
- Provide information, education and support to individuals and families living with high cholesterol, at risk for, or diagnosed with, FH, high Lipoprotein(a) and/or cardiovascular disease seeking help from The Family Heart Foundation to navigate their care
- Educate individuals and family members about the risks they may face, empower individuals to take an active role in their care, and identify resources to help them address those risks
- Offer tools and services developed by the Family Heart Foundation to support these individuals and families understand and navigate their particular diagnosis.
- Participate and work with the Family Heart Foundation Team to develop and refine educational materials to support efforts to:
- Connect individuals and caregivers with a specialist, using their interactive Family Heart Foundation “Find a Specialist” map and maintain the Specialist map by vetting, identifying and reviewing qualified clinicians
- Help individuals understand their diagnosis and treatment options
- Connect individuals to information about clinical trials and research opportunities they may qualify for
- Connect individuals and caregivers with peer support
- Advocate with payers to improve access to care on behalf of individual individuals
- Connect with pharmaceutical companies and individual assistance foundations to understand and explain available assistance programs for eligible individuals
- With the help of the Chief Medical Officer, stay informed on the latest research and guidelines related to high cholesterol, cardiovascular disease, FH, and Lp(a) and assist in updating materials to reflect changes
- Track individual requests and results to identify trends, prioritize needs, and report regularly on activities
- Understand privacy policies pertaining to individual communications
- Travel may be required
Skills and Experience:
- Baccalaureate or Master's Degree from an accredited institution, preferably in a Health or Human Services Field
- Minimum of 3 years of experience, preferably in a individual-facing role.
- Strong organizational, verbal (both English and Spanish), written (English and Spanish)
- Interpersonal skills.
- Must be able to demonstrate familiarity with word processing, spreadsheet, and web-based applications.
- Working knowledge of Salesforce is strongly encouraged.
Certification(s) and License(s):
- CCM, RN, PA-C, LCSW candidates preferred, though candidates without the specific credentials listed will be considered
