Collaborates with multidisciplinary team members to assess, inform, educate, and continually improve effectiveness and productivity of clinical and business programs.
Anticipates and identifies complex clinical and business processes and ambulatory issues, monitors and measures the risk factors. Communicates risk factors to informatics leadership and stakeholders. Manages projects to ensure the implementation of continuous quality control of process and deliverables.
Works with clinical staff to implement streamlined and efficient workflow changes (documentation, EMR tools, etc.).
Participates in planning and provides support for population health programs, including but not limited to, patient-centered medical home, transitions of care, health maintenance/prevention, and chronic disease programs, including those focused on diabetes, COPD, hypertension, etc.
Conducts data analysis to identify trends and patterns related to care outcomes. Develops action plans to address negative findings.

Qualifications
Bachelor’s degree in nursing. Master’s degree in Nursing Informatics in process, informatics or related field required.
Clinical licensure in the State of Maryland, or eligibility for licensure due to Compact state agreements is required.
Three years of clinical experience required. Two years working with Epic required. Prefer ambulatory clinical experience.
