    Job Description

    ** General Summary**

    Under the general supervision, provides care coordination, clinical utilization management and discharge planning for an assigned case load. This role is patient focused, outcome oriented and based on general and specialty professional standards and functions within an interdisciplinary practice model. This role is accountable for the continuity and integration of patient care services.

    ** Education and Experience**

    • Graduation from an accredited school of nursing is required: a Bachelor’s degree is preferred.
    • Licensure as a Registered Nurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the Maryland Board of Nursing, is required.
    • Three (3) years of Clinical nursing experience required.

    ** Knowledge, Skills and Abilities**

    • Highly effective verbal, written and interpersonal communication skills to communicate effectively with all levels hospital staff, patients, visitors, and levels of staff.
    • Knowledge of managed care payer mechanisms, case management, clinical utilization management and discharge planning is required.
    • Demonstrated ability to collaborate with multiple members of the health care and administrative leadership team
    • Ability to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served through knowledge of the principles of growth and development over the life span.
    • Excellent analytical and team building skills, as well as the ability to prioritize and work independently are required

    ** Physical Demands:**

    1. Moderate physical effort as it relates to transporting patients, assisting with transferring of patients, and carrying of patients medical records.
    2. Commonly utilized equipment include: typewriter, computer and printer, calculator, telephone, copy and facsimile machines.
    3. Sensory requirements:
    • Hear and participate in conversation/interactions amongst all members of the health care team
    • Ability to communicate verbally
    • Attend to detail and use CRT screen which may produce mental and visual fatigue.
    • Possess manual dexterity to operate equipment noted above.

    ** Working Conditions** :

    • Weekend, shift work, holiday, on-call, and overtime may be required.
    • Work is primarily performed in a standard office environment, but also requires contact and work with staff in a clinical setting.
