    Horizon Health in Howard, SD is seeking a Full Time Behavioral Health Support Coordinator. This position will play a key role in supporting our behavioral health providers by managing quality measures, coordinating patient resources, assisting with clinician caseloads, and providing overall support to ensure effective and efficient care delivery. The Support Coordinator will work closely with our therapists, psychiatrists, and other care team members to ensure seamless integration of care and improve the overall patient experience.     *Bachelor's degree in Social Work or related field is required.

    *This can be a remote position, Support Coordinator must be physically located in the State of South Dakota.

    Key Responsibilities:

    Manage Quality Measures

    Track, report, and ensure adherence to quality measures for behavioral health providers.

    Monitor and review patient records regularly to ensure required assessments and quality measure information is completed and up to date.

    Work with clinical staff to address gaps in care and enhance performance on quality measures.

    Coordinate Patient Resources

    Identify patient needs and connect them with appropriate community resources and support services.

    Assist with the referral process, ensuring timely follow-up and access to necessary care.

    Provide patients with information on available resources, including social services, transportation, and housing assistance, as needed.

    Support Behavioral Health Providers and Collaborate with Care Team

    Help manage clinician caseloads by coordinating schedules, ensuring timely follow-up, and minimizing gaps in care.

    Participate in case management meetings to discuss complex cases and ensure continuity of care.

    Assist with patient follow-up, ensuring patients are actively engaged in their treatment plan.
