We work in partnership with the Cook County Juvenile Court, DCFS, and other community providers to obtain referrals.
Under the direction of the Clinical Supervisor, the Clinical Counselor provides direct counseling services to adolescents and families. Complete all intake assessments, session notes, and other documentation as required by Medicaid and grant funders. Provide case management and service referrals to families as needed. Conduct community outreach and education regarding the program. Maintain client data as required by grant partners.
