Under the supervision of a Patient Care Supervisor, the Licensed Practical Nurse administers direct nursing care to patients and performs a variety of indirect patient care duties. The License Practical Nurse is responsible for all professional working policies pertaining to this section.
EDUCATION:
Graduate of an accredited Licensed Practical Nurse Program required.
A pharmacology course of at least forty-five hours must be included in the basic education
curriculum or have been attended post graduation.
WORK EXPERIENCE:
Home Health experience preferred.
Med-surg or rehabilitation experience preferred.
2 to 3 years experience as a LPN preferred.
Provides nursing care and prescribed treatments for patient’s in their home, according to physician’s plan of care as assessed by RN and in accordance with LPN practice act.
Evaluates patient’s physical, emotional and social needs and recommends alterations to the plan of care as indicated by changes in observed conditions.
Completes patient clinical record documentation, reflecting appropriate delivery of quality and safe home care services.
Performs related duties as required.
Completes nursing assessment forms in a timely manner. Communicates patient assessment to appropriate individuals.
Participates in patient care conferences, home health care staff meetings, appropriate committee meetings, home health care agency events and Quality Improvement Activities.
Refers patients to other health care in an accurate, complete and timely manner.
Promotes and maintains an agency environment in compliance with federal state and local regulatory agencies.
Participates in economical utilization of supplies and ensures that equipment is maintained in a clean and safe manner/environment.
Implements infection control and safety measures.
Communicates recommendations and requirements for additional home care services for patients to the appropriate individuals in a timely manner.
Participates in personal professional growth and development. Attends educational programs and review current literature regarding health care.
Implements current nursing practice to meet patient needs and changed conditions. Completes skilled nursing updates in a timely manner.
