    Horizon Health in Howard, SD is seeking a Full Time Care Coordination Manager. The Care Coordination Manager is responsible for leading and overseeing the care coordination team to ensure high-quality, patient-centered care. This role involves supervising nursing & non-nursing staff, developing workflows and procedures, collaborating with interdisciplinary teams, and optimizing care transitions to enhance patient outcomes. The Care Coordination Manager will also be responsible for compliance with healthcare regulations, staff development, and operational efficiency within the care coordination department.     Registered Nurse or Licensed Practical Nurse license is required for this position.

    Key Responsibilities:

    Supervise, mentor, and support care coordination team members.

    Establish and implement policies and procedures to improve care coordination programs and patient outcomes.

    Ensure smooth transitions of care between healthcare settings
