    Positions in this function include support of patients who receive services from provider practices participating in bundles with UNITEDHEALTHCARE through their health journey. Individuals in this role are responsible for patient management pre and post-surgical procedure to help educate on the digital platform, identify and remove patient barriers and connect patients to community resources and clinical programs. This role includes inbound and outbound communication to patients via telephone and digital platform. This position is full-time (40 hours/week) Monday- Sunday. Employees are required to have flexibility to work any of our 8 hour shift schedules during our normal business hours of (Monday- Friday 6:00 am to 9:00 pm and Saturday-Sunday 9:00 am to 8:00 pm). It may be necessary, given the business need, to work occasional overtime. Our office is located at 14141 Southwest Freeway, Suite 500, Sugar Land, TX 77478 .

    PRIMARY RESPONSIBILITIES:

    • Serve as an extension of the provider practice to assist in the end to end management of Medicare patients
    • Manage patient cases pre-surgery through post discharge and be the patient's go-to resource for non-clinical support along their medical journey
    • Monitor patient responses to daily check-in questions via a digital platform in order to help patients remove non-clinical barriers and connect with resources pre and post-surgical procedure
    • Serve as a Subject Matter Expert for the digital platform and educate patients on features/functionalities of the application
    • Work in multiple systems to access and document patient information and manage patient/provider communication
    • Outreach to patients needing individualized non-clinical support on their care journey
    • Answer inbound calls from patients or providers and address as appropriate
    • Provide excellent customer service to patients and provider practices
    • Participates in and contributes to staff meetings and other staff development opportunities and interdepartmental work groups
    • Work with Supervisor to identify system improvements that could be made to drive operational advancements and efficiencies
    • Assumes other duties as assigned by Supervisor

  • Qualifications

    REQUIRED QUALIFICATIONS:

    • High School Diploma/GED or higher
    • Experience using Microsoft Word (edit, create & save documents), Microsoft Excel (sorting & filtering data), and Microsoft Outlook (email, folders, attachments and calendaring)
    • Working knowledge of Medical Terminology

    PREFERRED QUALIFICATIONS:

    • Prior Healthcare Experience
    • Previous Clerical or Administrative experience
    • Experience working in a Call Center environment
