    We have a wonderful opportunity to join our Family Support Center as a Case Manager located in Burlington!

    Our Family Support Center offers a wide array of supports in the home and in the community for adults and children with intellectual and/or developmental disabilities living with their families. These services may include respite, engaging recreational activities, provision of adaptive equipment, skill training, and intensive staff support when necessary.

    The primary goal of the Case Manager working within our Family Support Center is to enable people with disabilities to stay together with their families and to be welcomed, contributing members in their community. We help parents and caregivers provide assistance in their own home for a child or relative with an intellectual and/or developmental disability. Our program provides the individualized resources necessary to support each family’s efforts!

    Our ideal candidate is a positive influencer, has the ability to build and maintain strong relationships with the individuals and their families, and will bring new and creative ideas to the program!

    Schedule : 30 hours (with flexibility to work in the afternoons, early evenings, and/or on the weekends)

    Pay Rate : $21.40/hour

    Required Skills

    • Excellent written and verbal communication skills (in the English language), including clearly documenting individual progress and activities in program records
    • Basic computer fluency, including Microsoft Office
    • Bilingual in Spanish preferred
    • Demonstrated ability to organize, delegate, and motivate others, as well as to work as a member of a team and to function independently
    • Sensitivity, caring, patience, and understanding of individuals with disabilities and needs of their families and/or caregivers

    Required Experience

    • High school diploma; Associate’s or Bachelor’s Degree in a related field preferred
    • Three years of related experience
    • Valid driver’s license and regular access to a reliable vehicle
