SALARY RANGE: $45K - $48K ANNUALLY, PLUS BONUS
JOB SUMMARY:
Comfort Keepers is seeking a well-spoken dynamic individual with great people skills to conduct in home assessments of potential clients for the purpose of converting the lead to a client. The candidate should have a background in home or health care/ Sales experience is a plus.
QUALIFICATIONS
- Must be able to work from car and visit various facilities such as hospitals and doctors' offices and the home of potential clients as well as our office.
- College degree preferably in Health Care, gerontology, or Marketing Management and two years related experience and/or training
- Ability to complete CSA within 1 year of employment
- Requires proficiency in word processing and computer skills (Office, Excel, & Power Point)
- Will master additional software including Google Calendar and eRSP management system
- Must possess and demonstrate excellent communication, human relations, and organizational skills as well as positive professional, business image and dress for success
- Must be a team player and work cooperatively with other members of the management team
- Responsibilities
- Under the direction of the Director of Operations, Client Care Coordinator performs intakes, client assessments and creates plans of care for new clients
- The CCC also contacts, presents, and develops referral relationships to ensure the best care for our clients by sharing the plan of care, assessment, and guide to care; giving an understanding of what collective team is doing for our clients
- Works with the entire office and caregiver staff to coordinate care for our clientele
- Interfaces with various referral sources and partnering with care providers to coordinate care and further develop those relationships
- Responsible for data entry that develops reports for the Director of Operations on a weekly basis, discussing successes and opportunities, and revisiting KPI's and goals
- Participates in management on-call rotation as necessary
- Performs assessments/in-home sales calls to establish services for potential clients
- Implements contracts and agreements to ensure sales goals are met
- Develops and maintains relationships with prospects, clients, care providers and referral sources database for relationship management
- Responsible for entering contacts, visit information (notes), and activity documentation in agency management system
- Follows up with prospects and clients, as well as referral sources on consistent basis, to provide best possible client coordination experience
- Tracks and reviews all efforts with DOP weekly using KPI reports
- Attends weekly management meetings and participate in the development of systems and procedures to improve care coordination, client satisfaction, and employee performance
- Supports office operations as needed
- Assists the other members of the management team as needed
- Is responsible for field training, spot checks, initial meet and greets
- Conducts scheduled and unscheduled supervisory visits for clients and caregivers
- Participates in on-call rotation by carrying the agency's emergency cellular phone
- While on-call, handles all client, caregiver, and scheduling issues that occur and conducts new client intakes as needed
- Is responsible for covering caregiving shifts in the event that a caregiver cannot be located to staff it
- Organizes and supports a wide variety of sales and marketing activities like event planning and ordering marketing materials including promotional items, attending fairs and other community events
