    Job Description

    General Summary

    Under limited supervision, the discharge coordinator is responsible for assisting in the implementation of the discharge and aftercare plans on the impatient psychiatric unit. The coordinator is a member of the care team and support team and has accountability to function as a part of the team to accomplish patient outcomes identified in the pathway/ plan of care.

    Reporting Relationships

    Reports directly to the Psychiatric Social Work Supervisor.

  • Qualifications

    Qualifications

    • Bachelor's degree in Social Work, psychology, or related field preferred. A.A.
    • Degree in related field with one year experience in medical/ psychiatric setting required. Currently licensed or license eligible in the State of Maryland when applicable.
    • If eligible, must sit for examination at next available time it’s given.
    • Knowledge of Microsoft Windows or successful completion of a hospital training program required. Knowledge of DSM-IV as appropriate.

    Working Conditions

    Works inside a clean. Well-lighted environment. May be exposed to multiple crisis situations. Time will be spent in office environment and in patient care areas. May be exposed to verbal abuse by patients. Expected to work overtime to complete a tasks that cannot wait until the next working day. Potential for minimal exposure to communicable diseases and hazardous materials, requiring the observance of Standard (Universal) precautions and safe handling practices.

    Physical Requirements

    • Standing, bending, ascending/ descending stairs, reaching, lifting, and pushing/ pulling is required 0-3 hours/ shifts.
    • Working, sitting, carrying is required 3-5 hours/ shift. Weight handled is less than 10 pounds.
    • Near visual acuity is necessary for accurate reading. Talking and hearing are necessary for telephone conversations and consulting patients/ families, communicating with staff members and visitors.

    Compensation:

    Pay Range: $19.5-$27.31
