  • Responsibilities

    Job Description

    Collaborates with an interdisciplinary team to assess care plans, monitor progress, and make recommendations to ensure safe and efficient hospitalizations. Responsible for evaluating patients in the ED for medical necessity and care coordination needs. Assesses potential admissions and works with advanced practice clinicians/physicians to determine appropriate admission status. Proposes and implements outpatient plans for safe discharge from the ED with physician approval. Coordinates care for patients with abuse, chronic pain, and frequent utilization issues. Orients new team members. Provides Observation Notifications to Medicare patients in Observation Status in the Observation Unit. Interacts with patients or customers of all ages, from newborn to geriatric.

    Qualifications

    Education & Training:

    • Must hold a current Maryland RN license or a Master's in Social Work (LMSW/LCSW).
    • A Bachelor's of Science degree in Nursing is preferred.

    Work Orientation & Experience:

    • At least 3 years of acute care experience required; 5 years for Social Workers.
    • Basic computer skills are necessary.
    • Emergency Department (ED) experience is preferred.
    • Experience in utilization management, case management, and discharge planning is preferred.

    • Demonstrates proficiency in clinical case management, conducting comprehensive assessments, and utilizing effective critical thinking skills both in writing and verbally.
    • Exhibits age-appropriate interpersonal interactions with patients ranging from newborns to geriatric adults.

    Abilities:

    • Communicates and collaborates effectively with internal and external customers, including colleagues, medical staff, liaisons, and patients/families.
    • Assesses, adapts, and responds calmly to changing or crisis situations.
    • Makes independent decisions in line with current policies, procedures, and ethical standards.
    • Prioritizes work assignments and manages time efficiently to complete tasks.
    • Assists in data analysis.
