Family Team Facilitator – Juvenile Justice Department

Monday – Friday (8 hours/day)
Position Overview
The Family Team Facilitator plays a vital role in supporting youth and families involved in the Juvenile Justice system. Using a wraparound approach, the Facilitator collaborates with families, youth, and their support networks to identify needs and develop personalized care plans.
Key Responsibilities
- Facilitate meetings with youth, families, and support networks to create care plans.
- Conduct strength-based assessments to identify needs and resources across all life areas.
- Build strong relationships with youth, families, caseworkers, and support systems using effective interpersonal skills.
- Deliver care that is strength-based, family-centered, and culturally responsive.
- Track service progress and maintain accurate electronic data for quality assurance.
- Lead family team meetings, complete assessments, and advocate for youth and their families.
Qualifications
- Education/Experience:
- Bachelor's degree OR 3–4 years of experience working with high-risk youth in the Juvenile Justice system.
- Skills and Training:
* YASI (Youth Assessment Screening Instrument) training preferred.
* At least 2 years of experience coordinating care and working directly with youth and families.
* Strong interpersonal and communication skills.
* Ability to work flexible hours, including evenings and weekends, to accommodate families' schedules.
* Exceptional attention to detail in service coordination.
