Job Title: Home Care Coordinator FLSA Designation: Hourly Hours: 40 Hours per Week; M-F 8am-4:30pm; after a training period, this job requires on call evenings and occasional weekends on rotation. Reports to: Franchise Owner Job Summary Home Instead Senior Care is seeking a self-motivated, reliable, and highly-organized In Home Care Coordinator who possesses a passion for seniors. Our Care Coordinator is a key role that matches staff care companions with clients and caregivers. You will be responsible for tracking client and staff change. You will work closely with clients, families and staff to meet the needs of both clients and caregivers. This position if heavy telephone work with constant customer and staff interface and computer input. You will work both independently and with a team of other professionals. The ideal candidate can manage their own workload, prioritizing tasks, and thinking ahead to meet the needs of clients and caregivers. Responsibilities and Duties - Answer multiple calls daily dealing with client and staff questions - Performs initial and ongoing in-home evaluations, direct care staff introductions and supervision of services. - Help in the process of hiring and maintaining direct care staff - Ensures timely completion of onboarding of new direct care staff according to procedure - Maintain direct care staff training hours - Maintains training tools that are assigned by the Franchisor and training to meet state standards - Assigning and monitoring work schedules of direct care staff providing home care - Communication with clients, family members, direct care staff and other professional involved in the care of the client - Perform administrative office tasks required to effectively carry out the responsibilities of the position including processing telephony for accuracy - Enters and maintains and corrects client data into the computer tracking system - Understand and adheres to established policies and procedures based on State Licensing - Handles Coordination of care for client services - Reports changes in client condition to the RN, Franchise Owner and other employees involved in direct care of client - Evaluate care plans, abuse prevention plans and other required documents; completed with intake. - Participates in the quality and assurance reviews and evaluation of the agency's services. - Uphold a strict level of confidentiality - Other duties as assigned Qualifications and Skills Essential Qualifications: - Exceptional interpersonal skills and high emotional intelligence - Exemplary planning and time management skills - Demonstrate ability to lead and develop a program and program staff - Ability to speak in front of groups and lead training sessions - Strong client focus, assertive, decision making skills, articulate, and creative in problem solving - Must be a licensed & insured driver - Demonstrated ability to manage multiple tasks, meet deadlines, and stay organized while providing an acute attention to detail Preferred Experience - Experience providing great customer service on the phone and in-person Preferred Knowledge, Skills and Abilities - Excellent communication skills, both verbal and written, including strong editing skills - Ability to use Microsoft Office, Outlook and Excel - Ability to work in a small open office setting unsupervised and with regular interruptions Physical Requirements - Hear and speak to use the telephone and communication with people - Work at a desk for extended periods of time - Able to travel to locations/homes in St. Louis, Carlton and Douglas counties on a regular basis - Move around the office to support staff, perform various tasks and room set-up
