R2R Palliative & Hospice care is looking for an excellent Hospice RN in the DFW area. Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.

• Conducting routine visits, admissions visits and/or follow up visits at home or other facilities setting

• Coordinating, assessing, and implementing patients' care

• Providing accurate and timely documentation of patient services to reflect plans of care

• Attending bi-weekly interdisciplinary meetings (only as needed/requested)

• Flexibility to be available days, evenings and weekends

• Bachelor's degree in nursing or health care preferred.

• Minimum two (2) years’ experience in hospice setting preferred

• Minimum one (1) year experience as a supervisor, preferred but not required.

• Must be computer proficient in typing and various programs, including background in EMR.

• Ability to work with and supervise others as an effective team builder and team player.

• Skilled at establishing/maintaining working relationships with key staff, marketing contacts, and clinical staff to ensure thorough understanding of Altus Hospice services.

• Ability to effectively evaluate performance and take corrective actions when necessary.

• Ability to work autonomously, take initiative, set priorities, organize work, and make independent decisions.

• Excellent communication skills, including public speaking, both verbal and written.

• Subject to criminal background check and drug screening.

• Holds current valid Texas driver’s license.

• Holds current, unencumbered license in the state of Texas as a Registered Nurse.

R2R Healthcare (Rainbow To Rainbow) is a licensed non-medical agency providing Personal Assistant Services (PAS) for the Dallas/Fort Worth area. Services are customized for the client and are available from 2 to 24 hours a day, 7days a week including holidays. R2R provides free in-home assessment of needs. Payor sources include Private pay, Insurance, and Medicaid.

Our individualized services are delivered by a team of skilled well trained staff who provide compassionate care and support in a responsive and timely manner. R2R also offers supplemental care to home health agencies and hospice.
