Provides crisis intervention, emotional support, counseling, education and advocates for patients/families. Collaborates with the interdisciplinary team in development, implementation and evaluation of the treatment plan. Typical Duties: * Conducts psychosocial assessments for patients/families with an irreversible serious illness. * Participates in the development, implementation and evaluation of patient treatment/care plans. * Collaborates with patients/families, clinical staff and palliative care team to identify options and goals. * Attend scheduled palliative care IDTs and team meetings. * Maintain current community resource information regarding services available for palliative care patients/ families. * Maintain clinical competence for adult/geriatric palliative Social Work services and exhibits professional growth and development. * Participate in quality improvement initiatives. * Maintain electronic medical records with accuracy and in compliance with JPS standards. * Performs other related job duties as assigned. Qualifications: Required Education and Experience: * Masters Degree in Social Work from an accredited university. Required Licensure/Certification/Specialized Training: * Licensed Master Social Worker (LMSW) by the Texas State Board of Social Work Examiners. * Cardiopulmonary Resuscitation (CPR) Certification; must be obtained within 120 days from date of hire. Preferred Education and Experience: * Experience working with life-threatening conditions, chronic disease and end of life care. * One (1) year experience in palliative care and/or hospice care.
