SUMMARY
The M.D.S. Coordinator Licensed Vocational Nurse (LVN) coordinates the minimum data set or M.D.S. which
documents residents needs in all aspects of their care and provides prescribed medical treatment and personal care to patients that are ill, injured, convalescent or disabled per the State of California Nursing Practice Act under the supervision of the Registered Nurse in the following settings at Mountains Community Hospital: Medical Surgical Unit, Emergency Services, Obstetrics, Perioperative Services and Skilled Nursing Facility. Upon termination of employment from this position a minimum of 60 days
is required to allow for adequate training and replacement to qualifications.

EDUCATION/EXPERIENCE
Graduate of an accredited school of nursing.
Preferred one-year clinical experience as an LVN in an acute care or skilled nursing facility.
CERTIFICATES, LICENSES, REGISTRATIONS
Current license to practice Vocational Nursing in the State of California.
Current CPR certificate.
ACLS desired.
