This individual will also complete performance documentation and address problems in a timely manner. DUTIES: * Guide and direct contract providers in the engagement of clients and their guardians through a structured collaborative relationship. * Guide and direct contract providers to provide for all medical needs of the individuals served. * Promote a person-centered approach to maximize independence, as well as improve and maintain lifelong wellbeing and functional status in the placement process for new individuals. * Guide and direct contract providers in practices that promote a high quality of care to individuals with developmental and/or intellectual disability. * Guide and direct others in addressing functional challenges of the individuals served and provide ongoing support and technical assistance. * Assist contract providers in identifying and addressing risks and current or potential gaps in care in order to prevent unnecessary deterioration that could result in hospitalizations, emergency room visits, or loss of independence in vocational, academic, or social functioning. * Complete necessary documentation to adequately monitor contract services. * Participate in the development of an individualized interdisciplinary plan of care for individuals in contract services. * Provide IDD Care Coordination as needed and as qualified. * Complete initial, annual, and other mandatory trainings, as assigned, related to monitoring IDD contractors' performance. * Ensure that all contract providers are complying with all agency and State regulations, policies and procedures. * Collaborate with all members of the client care teams and other agencies regarding IDD contract services. * Maintain HIPPA compliance and appropriate client confidentiality. * Educate contract providers regarding completion of all required paperwork and documentation. * Monitor documentation and time of contract providers and address problems in a timely manner. QUALIFICATION REQUIREMENTS: * Bachelor's degree in acceptable behavioral health field of study. * Minimum of one year of experience in a community-based developmental disabilities services. * Completion of required trainings. * Ability to drive a motor vehicle and a valid driver's license. *Erigo Employer Solutions is committed to compliance with all applicable EEO laws and hiring methods.
