    Horizon Health is seeking applicants for a Full Time Patient Care Coordinator who will act as an extension of the primary care team by supporting and leading care coordination efforts across multiple programs and initiatives. The position supports primary care providers by providing patient outreach on key quality initiatives. We are looking for candidates who are passionate about delivering high-quality, patient-centered care.     Must have South Dakota Registered Nurse or Licensed Practical Nurse License.

    Can be fully remote for the right candidate.

    Key Responsibilities:

    Provide oversight to the Medicaid Health Home program, including education of staff and patients, submission of reports, and development of program metrics and goals to measure success.

    Provide care coordination services for patients enrolled in Chronic Care Management, including education to staff and patients, submitting required reports, and development of program metrics and goals to measure success.

    Provide care coordination services for patients participating in Maternal Health initiatives, such as prenatal, postpartum, and infant care programs.

    Provide care coordination services for new programs built at the organizational or state level, including education of staff and patients, submitting required reports, and assists in development & achieving program metrics.

    Work collaboratively with clinic teams to support improvement on key quality initiatives, including child immunizations, cancer screenings and other priorities as identified.
