    Position Summary:
    We have an exciting opportunity to join our team as a Pharmacist - Per Diem Variable Day/Evening/Night shifts; Rotating Schedule/Weekends - Pharmacy - Pha.

    In this role, the successful candidate Under general supervision compounds dispenses medication and other pharmaceutical supplies according to established operational and chemical procedures as prescribed by qualified prescribers Reviews and interprets prescriptions obtains clarifications as necessary and regularly consults with medical staff nursing personnel and others in order to coordinate delivery of a high pharmaceutical service to hospital patients and selected outpatients Oversees and closely reviews the work of technical and clerical support

    Job Responsibilities:

    Clinical Skills:

    * Reviews written prescriptions to determine patients needs due to condition with no anticipated reaction
    * Advises physicians and nurses on dosage adjustment drug interactions drug/lab interactions high risk patients incompatibilities side effects and instructions for drug administration
    * Attends continuing education programs and reads professional literature to keep current of developments in the field and to maintain licensure
    * Participates in development and implementation of patients education programs pharmaceutical care and lecture programs for nurses other paramedical personnel and patient counseling
    * Assists the Clinical Pharmacist in the development of medication use processes
    * Knowledge of clinical intervention nbsp; based on the specific needs of the patient population considering patients age growth and development medical condition physical abilities psychological status potential language barriers religious beliefs and level of comprehension
    

    Functional Skills:

    * Contacts physicians as necessary
    * Prepares or oversees preparation of prescriptions as ordered by prescriber
    * Compounding medications according to established procedures
    * Ensures labels are generated and affixed to containers to identify data and directions in accordance with hospital and/or government regulations
    * Prepares intravenous admixture solutions using aseptic technique ensuring compatibility and stability in accordance with prescriber rsquo;s specification
    * Ensures all balances and other like equipment are maintained in an orderly manner and aseptic techniques are followed
    * Ensures nursing stations have appropriate supply levels as well as periodically checking for outdated or overstocked medications
    * Ensures proper storage and handling of medications
    * Participates in the proper standards of preparation of chemotherapy and hazardous medications
    * Contributes equally to the overall quality and quantity of work produced by the department in proportion to hours worked
    

    Clerical Skills :

    * Documents all consultations in the Pharmacy Consultation Log
    * Maintains records pertaining to drug and supply ordering storage and distribution as required by hospital and/or government regulations
    * Prepares documentation necessary for preparation of patient charges pharmacy operation narcotic control records and patient medication profile
    * Maintains complete and accurate records of dispensing patient medication profile cart fill piggyback lists narcotic control forms IV room maintenance log etc
    * Ensures all equipment reference materials work areas and the like are maintained in a neat and orderly manner
    * Performs formal monthly inspections of nursing units and assigned Pharmacy areas for outdated drugs
    

    Professional/Interpersonal Skills

    * Assists senior personnel in overseeing and reviewing work of interns and technicians
    * Participates in the training and education of pharmacy students and interns patient education programs lectures to nurses etc as required by supervisor
    * Communicates with other staff members in order to maintain continuity in handling drug/prescriber related problems or information
    * Organization of workload to meet deadlines and priorities
    * Flexibility to redirect daily activities based on departmental needs
    * Displays a progressive attitude with the ability to accept constructive feedback and integrate into performance
    * Accepts responsibility for one rsquo;s actions
    * Identifies and offers solutions to problems
    * Assumes ongoing responsibilities to ensure the smooth operation of the Pharmacy Department
    * Participates in and supports collaborative decisions made by management
    * Demonstrates a working knowledge of nbsp; policies and procedures specific to the assigned pharmacy service
    * Maintains knowledge base through participation in learning situations such as departmental meetings educational programs professional association membership journal review or committee participation
    * Fulfills the annual requirements regarding continuing education
    

    Minimum Qualifications:
    To qualify you must have a Pharm D or BS in Pharmacy required Ability to walk and/or stand for 85 of the work time Knowledge of clinical intervention and the ability to meet the specific needs of the patient population Knowledge of and in depth understanding of pharmaceuticals to prepare and dispense medications Three to six months on the job experience Good interpersonal and communication skills Detail oriented with a high level of analytical ability.

    Required Licenses: Pharmacist License-NYS

    Qualified candidates must be able to effectively communicate with all levels of the organization.

    Long Island Community Hospital provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $78.62 - $78.62 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
