Job Description
General Summary
Interviews patients, family, and caregivers
Collaborates with outpatient facilities and pharmacies.
Works along-side the inpatient care-team in order to document the most accurate and up-to-date list of medications a patient is taking at home.

Reporting Relationships
The Technician reports to the Clinical Pharmacy Coordinator and/or Pharmacist on duty. Job requires moderate level of supervision.
Qualifications
Education : High School Diploma or equivalent required.
Completion of technician training program preferred.
Licensure : Registered Pharmacy Technician with State of Maryland Required. Certified Pharmacy Technician preferred.
Work Experience : One – two years working experience as a pharmacy technician in a retail setting preferred, or prior experience performing medication history/reconciliation in an institutional setting.
