PURPOSE:
The HEALTHCARE DATA ENTRY SPECIALIST will work with the Patient Care Quality Advocate and Clinical Outcomes Analyst to close gaps in care for various HEDIS measures (chronic care and preventive health measures) for insurance companies and other internal quality projects. For patients with gaps in their care, they will make appointments for the patients as necessary. The HealthCare Data Entry Specialist will also be responsible for helping with the ER Hotline as instructed and contact and make appointments for these patients in a timely manner.
ESSENTIAL DUTIES/RESPONSIBILITIES:
- Communicates effectively and courteously on the telephone and in person with patients and colleagues
- Aware of compliance, appropriateness, and coordination of care for patient
- Assists in resolving financial issues that hinder patient’s ability to access care
- Promote healthier living and engage patients to achieve better health outcomes
- Educates patients on importance of preventative care and associated health risks and outcomes
- Helps manage chronic care management program
- Assists in publishing and distributing quality reports and helps with annual reporting
- Follow outreach protocols to schedule patients for necessary visits, labs, procedures, etc.
- Routinely employ fail safe system to verify patient attended scheduled appointments and intervene where necessary and appropriate
- Helps with referrals to other physicians
OTHER DUTIES:
- Assist with other ACO initiatives as assigned, such as coordinating high priority follow up appointments for patients deferred from ER

QUALIFICATIONS
REQUIRED:
- Currently enrolled in college
- Strong computer skills - knowledge of computer systems and applications such as EMR, practice management software, Microsoft Office and Excel
- Combination of education and administrative experience
- Available to work 15 to 20 hours a week during normal business hours
- Skill in organization and attention to detail
- Skill in using customer service techniques
- Knowledge of organizational policies, procedures and protocols
- Ability to understand the importance of accuracy and timeliness of assigned task
- Ability to demonstrate compassion and caring in dealing with others
- Ability to communicate clearly and accurately and work as a member of a team
- Ability to identify problems and implement solutions
- Ability to listen carefully to the patient needs
- Ability to communicate with physician and the patient
- Knowledge of common safety hazards and universal precautions to establish a safe work environment
- Ability to use departmental supplies and equipment in a cost-efficient, effective manner
- Skill in mediation and conflict resolution
PREFERRED:
- Experience in outreach and patient care coordination
- Knowledge of health care field and medical specialties
- Knowledge of current medical laws, rules, and policies
- Knowledge of medical terminology
- Experience in a medical office/clinic setting
- Extensive experience in customer service and patient interaction in medical field
- Familiarity with motivational interviewing techniques
- Experience scheduling patient appointments in medical office
