Job Summary
The Clinician provides comprehensive clinical and care coordination services to individuals enrolled in the Suicide Prevention Program. These services include conducting screenings, completing assessments, developing individualized recovery plans and providing individual, group, and family therapy. All therapeutic services implemented by the Clinician are informed by evidence-based practices (EBP), including the Collaborative Assessment and Management of Suicidality (CAMS) and Counseling on Access to Lethal Means (CALM), ensuring the highest standards of care.
Responsibilities
Conducts comprehensive assessments, including initial and ongoing evaluations of suicide risk, substance abuse, living situation, support systems, mental status, medical status, and other relevant factors.
Provides individual, group, and family therapy, utilizing evidence-based practices (EBP) such as the Collaborative Assessment and Management of Suicidality (CAMS) and Counseling on Access to Lethal Means (CALM), focusing on functional, social, personal, and recreational skills.
Conducts suicide risk screenings and provides comprehensive risk assessments, biopsychosocial assessments, and safety planning on the day of screenings.
Facilitates stabilization of mental health symptoms through ongoing assessment, outreach, and symptom management.
Provides assertive follow-up contacts based on suicide risk, ensuring timely face-to-face contact within 24 hours of missed appointments.
Completes mental health status exams on individuals in crisis. If licensed, provides assistance in patient emergencies and completes professional certificates for individuals meeting inpatient admission criteria.
Maintains current knowledge of the Florida Baker Act Statutes 394, forms, procedures, and other relevant regulations.
Collaborates with internal and external treatment providers to ensure comprehensive and collaborative care.
Participates in providing 24-hour on-call coverage as part of the program’s crisis response.
Engages in outreach efforts to crisis stabilization units, inpatient psychiatric facilities, domestic violence shelters, and emergency departments to identify, engage, and enroll eligible individuals.
Develops personalized care and treatment plans in collaboration with individuals served, their families, service providers, and significant others, addressing identified needs and leveraging individual strengths.
Coordinates and implements transition of care plans, including warm hand-offs for individuals exiting the program, and ensures engagement in follow-up care.
Advocates for and secures necessary services and resources to implement care and treatment plans, coordinating the delivery of services as specified in the plans.
Maintains regular contact with treatment providers, monitors service delivery, and evaluates the effectiveness and satisfaction of services provided.
Provides community-based outreach to service providers during critical points in the system to provide information on the program's services.
Qualifications
In order to be considered, candidates must have a Master’s degree in Mental Health Counseling or a related Human Services field from an accredited university or college (a related Human Services field is defined as one in which 30 hours of course work includes the study of human behavior and development) and experience working with adults with a severe and persistent mental illness required.
One year of experience working with individuals with mental illness required.
Valid Florida License (LMHC, LCSW or LMFT) preferred.
Certification in CFARS preferred.
Must demonstrate proficiency in the MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.
Proficiency of Microsoft Office Programs and the use of the Internet.
Strong communication skills are essential and this individual must be able to interact appropriately with internal and external customers, including patients, families, caregivers, community service providers, supervisory staff and other department professionals.
