Job Summary: Performing Intake Care including patient information collection, and documentation for new and existing patients, facilitation of patient flow, and facilitation of compliance of patient throughout treatment program designed around each individual in order to achieve long term goal of freedom from addiction.
KEY DUTIES, RESPONSIBILITIES AND SKILLS:·
Ordering, Organizing and Stocking of educational materials from manufacturers and other resources in collaboration with multidisciplinary team.
Initial Intake and Ongoing Patient Information Collection and reporting in a timely manner
Patient education and observance of induction medication timing
Assisting patients in creating realistic goals toward achieving freedom from addiction
Ensuring accountability of treatment program/patient compliance
Ensuring compliance with HIPPA and CFR 42 part 2 regulations
Patient and Family/Significant Other education – Including, but not limited to, proper and effective administration techniques, management of side effects, proper storage and protection of medication, recovery resources, support group, and counseling information
Urine Drug Screen review and collaboration with multidisciplinary team
State Medication Report (KASPER) review and collaboration with multidisciplinary team
Collaboration with healthcare providers outside of Horizon Health to assure continuity of care
Assisting with Physician education/compliance of company policies
Assist patients with referrals to community resources.
Excellent verbal and written communication skills
Ability to establish and maintain effective working relationships in a multidisciplinary team environment
Ability to work with compassion for others regardless of race, cultural background, sexual orientation, religious beliefs, political beliefs, or any other issue that would be a potential therapeutic barrier to helping someone with addiction issues
Willingness to continue training/education in addiction recovery and therapeutic regimens
Basic phone/computer/electronics skills (email, Microsoft Word, Microsoft Excel, Internet, Fax, Copier)
Ability to effectively present information and respond to questions from teammates, patients and other members of a multidisciplinary team
EDUCATION, SKILLS, CERTIFICATION AND EXPERIENCE·
Bachelor’s degree in a human services field, including: psychology, sociology, social work, family studies, human services, counseling, nursing or another human service degree program related to working with individuals with substance use disorders AND a minimum of one (1) year of full-time employment working directly with adolescents or adults in a human service setting after completion of educational requirements or a master’s degree in a behavioral science, as defined above, may substitute for the one (1) year of experience.
