    ABOUT THIS POSITION

    Our Referral Coordinators provide and manage the critical links between our patients and supportive organizations and other medical and dental professionals.

    JOB DUTIES AND RESPONSIBILITIES:

    Referrals

    • Link patients with needed medical and dental referrals
    • Prioritize and track all stages of a referral requesting reports and provider guidance
    • Assist patients in scheduling follow-up specialty medical and dental care appointments
    • Meet with patients to review referral procedures and requirements and address barriers to getting care and answering all questions
    • Document patient data in systems as needed
    • Cultivate relationships with community service partners, in particular the Coalition of Community Health Clinics and our partner safety net clinics.
    • Scan and fax records for enrollment in specialty care programs
    • Oversight and coordination of patient gap-care programs such as, but not limited to, Project Access Now, Screenwise, and EpicCares
    • Provide back-up support as needed to medical and/or dental reception services
    • Other duties as assigned

    Eligibility and Enrollment

    • Provide enrollment and eligibility assistance for patients to public and private assistance programs
    • Educate patients on using health care resources
    • Attend to any required reporting concerns
    • Track patients identified as food insecure and ensure they are aware of community food assistance, gardening, nutrition and cooking resources

    Wallace Team Member

    • Update policies and procedures as needed
    • Maintain strict patient confidentiality
    • Participate in Health Literacy events, volunteer retention and related activities
    • Comply with all applicable laws and fully cooperate with the Wallace compliance program
    • Participate in Continuous Quality Improvement initiatives
    • Perform other duties as assigned

    QUALIFICATIONS, SKILLS AND ABILITIES

    Required

    • At least one year of referral coordination experience in a medical or dental practice
    • Bilingual Spanish/English required
    • Excellent and sincere customer service
    • Excellent organizational skills and ability to prioritize the work independently and as a member of a team
    • Attention to detail
    • Desire to work with people of diverse ethnic, social, economic, and cultural backgrounds

    Preferred

    • Experience assisting clients with multiple barriers including poverty, language, mental illness or housing
    • Solid computer skills, including Microsoft Excel, Word, Outlook, and Access
    • HIPAA training and certification
    • Experience with OCHIN Epic electronic medical record system

    WORKING CONDITIONS

    Duties are performed primarily in an office and clinic environment.  Some evening or weekend hours may be required. 

    PHYSICAL REQUIREMENTS

    Ability to work in a fast-paced environment. Requires ability to sit, stand, lift, carry, walk, read fine print, and communicate in person, in writing, by telephone and electronically.
