- Servicing? Cobb Hospital POSITION SUMMARY: Provide management over a
team of nurses to assist with high level customer service,
communications with Hospital Leadership, assist nurses with problem
solving, provide education and teaching in the region.? Position
increases quantity of Corporate Compliance and maintain a high level of
compliance to policies and procedures. Position promotes the mission of
Committed to Caring. ESSENTIAL JOB FUNCTIONS, DUTIES, AND
RESPONSIBILITIES: * The transition nurse will function as a resource to
hospitals and patient care. * Bridge the gap between hospital stay and
next steps after surgery, injury or illness. * Assists patients in
providing the highest level of independence. * Assesses the needs of
each member, advocates as appropriate services for the patient. *
Establishes a trusting relationship with the member, their family and
other parties involved to facilitate access to needed services. *
Collaborates with care managers, multidisciplinary providers, benefit
specialists, behavioral health specialists, pharmacists, vendors, and
social services to enhance member satisfaction, conserve time and
resources, identify barriers, improve outcomes and reduce avoidable
readmissions. LICENSURE, CERTIFICATION, EXPERIENCE AND EDUCATION
REQUIREMENTS: * ? ? ? ? ? ? Registered Nurse with current active? state
license. * ? ? ? ? ? ? BSN from accredited school of nursing.? * ? ? ? ?
? ? 2 years' experience in skilled nursing, home health or hospice. * ?
? ? ? ? ? Work experience as a nurse liaison, case manager (hospital or
insurance), LTAC or Nursing Home care leadership. * ? ? ? ? ? ? High
energy, self-motivated individuals who can multitask and prioritize. * ?
? ? ? ? ? Strong management, communication, interpersonal, leadership,
organizational, and clinical skills. * ? ? ? ? ? ? The ability to work
independently is essential.? * ? ? ? ? ? ? Community Relations /
Customer Service experience is a plus. * ? ? ? ? ? ? Excellent clinical
