Population Health Specialist Vanguard Medical Group PA - Verona, NJ 07044 Description: We're seeking a motivated, energetic individual for a position which includes working independently, employing analytical and management skills, and creating supportive relationships with patients. This is a full-time position at Vanguard Medical Group, in the Verona, NJ location. We offer innovative primary care across all sites of our Family Practice organization, and the PHS is an essential member of our dynamic health care team. Although this isn't a hands-on clinical position, the PHS must have a good working knowledge of the health care field. Key Responsibilities: 1. Make outreach calls to patients regarding Emergency Room and Inpatient Admissions; schedule follow-up appointments; educate patients about alternatives to Emergency Room usage 2. Provide resources and assistance to patients who require clinical, insurance-based, and supportive care services 3. Facilitate patient appointments with Primary Care Provider, specialists, labs, and other healthcare entities; obtain referrals and insurance authorizations 4. Accurate data management using Excel spreadsheets for a variety of value-based programs 5. Perform outreach activities in association with clinical and utilization metric management 6. Participate in Population Health initiatives including managing outreach programs, mailings, and patient education programs 7. Attend trainings, meetings and presentations associated with the Care Coordination Department and the VMG organization as required 8. Perform administrative duties including copying, report collation, organizational tasks, and other duties as assigned by Director Job Essentials: 1.
