WWH specializes in lesbian, gay, bisexual, and transgender (LGBT) communities, people living with HIV, and individuals facing barriers to accessing care. Established in 1973, WWH provides holistic services focused on improving health and wellness, including primary medical and HIV specialty care, dental services, behavioral health services (mental health and addiction treatment), medical adherence nurse case management, public benefits and insurance navigation, legal services, HIV education, prevention and testing, support groups, and wellness services such as nutrition, yoga, meditation, and acupuncture. Job Summary The Community Health PrEP Navigator provides HIV, STD, and general health and wellness related education in one-on-one and group sessions, conducting specific outreach to targeted populations as dictated by programmatic needs. The PrEP Navigator provides support, education and guidance to clients at high-risk for HIV with the goal of supporting access and adherence to PrEP (Pre-exposure Prophylaxis) to prevent HIV infection. This person will follow-up with clients by phone and in person to try to engage them in services. The Community Health PrEP Navigator will report directly to the Director of Community Health and Wellness and will work in collaboration with the Medical Adherence Department, the Behavioral Health Department, Public Benefits & Insurance Navigation, and other departments throughout the health center. The Community Health PrEP Navigator will participate in Whitman-Walker Health activities as necessary, as well as any trainings, and will follow the direction set forth by Whitman-Walker Health. Primary Essential Duties Perform community outreach to work with other agency navigators to find and support activities to engage new consumers in PrEP services. Conduct educational sessions with targeted populations, providing HIV, STI and general health and wellness related information using age appropriate, culturally sensitive, agency approved materials/models and approaches. Educate patients referred for PrEP about PrEP services and facilitate PrEP appointments for those interested in/potentially eligible for PrEP. Offer in-person escorts and other support for the first PrEP appointment. Work with the Director of Community Health and Quality Improvement and the Informatics Department to develop and implement data collection protocols and internal and external PrEP referral protocols for high-risk HIV negative individuals. Conduct HIV testing and STD testing for clients in both clinic and mobile-testing settings, providing HIV prevention counseling, referring to appropriate programs and Whitman-Walker Health providers, and coordinating logistics of initial medical appointments for all HIV-positive individuals. Assist the new consumers in overcoming barriers to accessing PrEP, primary medical care and support services. Educate patients on self-advocacy skills and empower them to communicate effectively with their health care team. < >.< >. Assist with monthly reports and participate in meeting with stakeholders. CTRS referrals: Work with patients referred from Clinical Testing and Referral Services (CTRS) to help engage and retain in PrEP care through counseling, education, and navigation. Insurance Navigation referrals: Assist potential patients with initial insurance navigations and referrals to the Public Benefits and Insurance Navigation (PBIN) department for insurance inquiries and setup < >: Assist patients who have been prescribed Truvada for PrEP and who need financial assistance to obtain their prescription through various channels, which include the Patient Assistance Program & Co-pay Assistance Program through Gileads Patient Advocacy Foundation and others. On call for Providers: Work in coordination with Whitman-Walker Health medical providers to assist patients in need of financial assistance and navigation on an on-call basis. Coordinate with the pharmacy: Work in conjunction with Whitman-Walker Health Pharmacy to troubleshoot and assist patients issues as needed. Assist the PrEP clinic: Work with patients referred from the PrEP Clinic who are experiencing adherence challenges. Assist in other matters relating to the PrEP Clinic when necessary. Adherence follow-ups: Assist patients retention in PrEP care through adherence counseling to maintain and increase patient adherence and decrease potential lapses through routine telephone or in-person follow-ups < > Knowledge of ambulatory healthcare, PEP/PrEP, HIV, STIs. Ability to conduct sensitive, empathetic interviews that respect the dignity and diversity of clients. Experience working with MSM of color and Transgender individuals. Ability to maintain a respectful, nonjudgmental, and compassionate manner with WWH clients. Ability to maintain client records and information in an accurate, timely, and confidential manner. Excellent face-to-face interpersonal skills, with the ability to effectively interview, assess, educate, and counsel clients from diverse educational, social, and economic backgrounds. Well-honed analytical skills, including the ability to understand, interpret, and present data related to the tracking of various Community Health related events, diseases, and behaviors. Well-developed organizational skills, including the ability to multitask and the capability to consistently reprioritize work as necessary to meet deadlines. Flexibility of schedule shifts include regular evening and occasional late night and weekend hours. Knowledge of HIV/STD risk factors, techniques for reducing the risk for HIV/STD transmission, methods for conducting HIV/STD counseling and testing, methods for counseling clients about test results, linkage to treatment, and support services. Knowledge oftechniques for assessing clients needs, developing risk-reduction and referral plans, identifying resources, making referrals, following up to ensure completed referrals, documenting client interactions, and proactively supporting clients pursuit of goals. Understanding the needs of diverse communities and their interactions with the clinical systems that serve them. Familiarity with local neighborhoods. Strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interactions with diverse groups of co-workers, external business partners, and the community. Ability to work in a multidisciplinary team structure by demonstrating ability to collaborate and contribute to the teams work. Adherence to all Whitman-Walker Health policies and procedures. Experience working with eCW or a similar electronic client database is preferred. A 37.50-hour work week is a general requirement, with a flexibility of schedule shifts may sometimes include evening and occasional weekend hours. Some early morning and late evening coverage may be required depending on scheduling requirements. This position will be located at 1525 with some coverage requirements at MRC, Mobile, and Eastern Market. Education and Experience Required 2 years of direct experience or Bachelor's Degree in public health, psychology, social work, or related field. Fluency in Spanish required. Fluency in Medical Terminology preferred. HIV testing certification from BioLytical (INSTI) preferred; experience providing risk reduction counseling preferred. < > Customer service experience and training preferred. Experience in a medical center and/or non-profit environment preferred. Working Conditions: Working conditions for this position are normal for an office environment. Individual will be expected to work occasional evenings and/or weekends and must be available to staff events outside of the office environment.
