Incumbent serves as a single point of contact for patients and/or family members for the purpose of effectively communicating the patient's plan of care, and coordinates care with the physicians and members of the healthcare team. Interacts with foundations, professional associations, medical professionals, etc. to promote the goals of education and community access. 20% Develops, modifies and implements a plan of care that is specific to the disease, age, and needs of the patient. Coordinates with Medical Center personnel to effectively and timely implement the plan of care, including scheduling initial consultations, referrals, follow up appointments, ancillary services, etc. Delegates patient care activities to appropriate members of the health care team, provides effective leadership and ensures that care is safe and of high quality. Serves as an advocate for the patient during the development and implementation of the plan of care. 50% Serves as a clinical expert relative to urology, providing education and consultation services to the clinical staff. Assists in triaging of patients. Conducts effective teaching sessions for nursing staff, medical trainees, etc. on the subject of urology. 10% Serves as a single point of contact for patients and/or family members for the purpose of effectively communicating the patients plan of care.
