    FULL JOB DESCRIPTION

    1ST ALTERNATIVE SUPPORTS & SERVICES - We are hiring two waiver support coordinators to provide support coordination services. One full-time support coordinator is needed in the Hillsborough area, and one part-time support coordinator is needed in the Pinellas area. The ideal candidates must reside in either location appropriate to our client's locations to complete the required in-person visitation with our clients. When the support coordinator is not visiting clients, they work remotely at home to complete support coordination tasks through our company.

    Support Coordination is the service of advocating, identifying, developing, coordinating, and accessing supports and services on behalf of individuals or assisting individuals and their families in accessing supports and services on their own.

    Qualifying disabilities for the Medicaid Waiver program include intellectual disability, Autism, Cerebral Palsy, Spina Bifida, and Prader-Willie Syndrome. Serious inquiries, please respond with a cover letter and resume.

    MANDATORY WAIVER SUPPORT COORDINATOR QUALIFICATIONS:

    • A bachelor’s degree from an accredited college or university
    • Possess two years of paid, supervised experience with developmental disabilities, mental health, counseling, social work, special education, or/and rehabilitative services.
    • Must pass a level 2 background screening, a local law clearance, possess a valid driver’s license, and maintain automobile insurance.
    • A master’s degree in a related field can substitute for one year of the required experience.

    RESPONSIBILITIES AND DUTIES NOT LIMITED:

    • The position allows for a highly flexible work schedule as a waiver support coordinator works primarily from home and manages their schedule based on the needs of their clients.
    • The Support Coordinator supports individuals with developmental disabilities in accessing resources and coordinates needed supports and services in a community setting.
    • Assists the individual with developing a Person-Centered Support Plan with the individual and their circle of support, family members (natural supports), and other supporting parties in the individual’s life.
    • Maintain an individual record for each client you are rendering services to.
    • Assist in obtaining the needed support and services to achieve desired outcomes through paid and non-paid resources in the community.
    • Advocate for consumer rights, encourage and educate consumers to self-advocate and make the best decisions to their ability.
    • Maintain all expenditures within the approved cost plan and monitor all services for appropriate usage per Florida Medicaid Developmental Disabilities Individual Budgeting Waiver Services Coverage and Limitations Handbook.
    • Waiver support coordinators are Responsible for all administrative functions of support coordination activities and perform all other duties as assigned and needed through the agency.
    • Participate in required meetings and training.

    WORK SCHEDULE: Can be Mon-Fri, 9 am -5 pm, but flexible upon deadlines and scheduled visits with clients, providers, training, and team meetings; however, on-call 24/7 in the event of an emergency.

    PAY: The pay for a full-time caseload is 100.00 per full case and 50.00 per limited case, not exceeding over 43 clients. Additionally, a monthly stipend for travel will be discussed during the interview.

    WORK LOCATION: Both remote and mobile to complete required documentation and mandatory home and community visits.
