Job Description: Apex Home Health Agency is seeking RNs/LPNs to provide home health care services at the patients’ home. We have an immediate need for RNs and LPNs to provide home health care in Orange and Seminole Counties. Home Health nurses travel from one patient’s home to another to provide Skilled Nursing services such as:

Wound Care

Intravenous Medication Administration

Midlines/PICC lines dressing changes

Assess and observe patients’ condition at each visit and document in a timely manner

Comprehensive assessment including reviewing medication and vital signs

Provide education to patients and caregivers

Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patients’ care team

Skills:

Strong interpersonal skills

Critical thinking skills

Basic computer knowledge

Ability to commute

License/Certification:

Florida RN license or

Florida LPN license

BLS Certification

Experience:

6 months to1 year nursing experience preferably home health experience

New graduates are highly encouraged to apply

Weekly schedule:

· Monday to Friday

· Rotating weekends

· Weekend availability
