    Access Medical is a nurse owned and operated agency located in Corrales, NM and we are growing! We are seeking a compassionate Team Care Coordinator to join our team of professionals who make a difference in our patient’s lives.

    Job Summary: The Teams Care Coordinator assists with the day-to-day operations of the Coordination of patient and staff visits, scheduling, IDG meeting scheduling, communication with field staff, and back up to medical records. Provides a variety of administrative and clerical tasks.

    Qualifications:

    1. Be eighteen (18) years of age or older.

    2. Education or experience requirements: Two years experience in healthcare and or medical industry preferred or equivalent office manager experience.

    3. Knowledge of office management systems and procedures.

    4. Demonstrated knowledge of computer and internet applications.

    5. Demonstrated ability to organize efficiently and establish priorities for job functions.

    6. Demonstrated excellence in both written and oral communication skills including spelling and grammar.

    7. Demonstrate ability to handle stressful and delicate situations with tact, diplomacy, and attention to their importance.

    8. Demonstrated ability to represent agency in a positive and professional manner.

    9. Must have a valid NM driver’s license.

    10. Must have reliable transportation available during work hours. Vehicle must be properly registered and insured in accordance with state law.

    Duties and Responsibilities:

    1. Answers and directs incoming phone calls to appropriate team members.

    2. Back-up for office staff for lunches and out of office.

    3. Back- up for medical records

    4. Maintains the medical supply closet.

    5. Work with Clinical Directors for any staff changes updates in reference to discharges, visit schedules and clinical meetings.

    6. Attend department meeting on a regularly scheduled basis.

    7. Adhere to dress code.

    8. Adhere to work schedule and maintain good punctuality and attendance record.

    9. Assemble Admission Packets

    10. Handle Primary care office duties such as: managing multiple schedules, call staff and providers

    11. Work on and follow up on orders

    12. Run eligibility for patients

    13. Perform other duties as assigned.

    Regulatory Requirements:

    1. Satisfactory completion of background check in accordance with Sate of NM Caregivers Criminal History Screening Program.

    Physical or Environmental Demands

    1. Light to Moderate: Some bending stretching, handling supplies, and occasional lifting up to 50 lbs. Must be able to drive in all types of weather.

    Expected Work Hours:

    1. Varies by agency needs; Typically, Monday through Friday 8 am to 5 pm. Other hours required as needed and assigned.
