Nurse Practitioner

Our client is a growing healthcare organization based in Spring Valley, NY.

The Company: Our Client provides patient-focused precision medical care to patients in assisted living facilities and is committed to providing patients with the highest quality medical care.

The Position: Our Client is looking for a Family Nurse Practitioner.

• 40% of Medicare Fee Schedule
• Ability to Make Additional Compensation
• 1099 Position
• Potential Sign on or Relocation.

Requirements:

• Board Certified Nurse Practitioner
• Experience in Geriatrics
• 2 Years’ Experience
• Active New York License
• Minimum 4 hours in facility (can chart from home)

Responsibilities:

• 3-4 Days a Week (No Weekends)
• Flexible Scheduling Either 9am-1pm or 9am-2pm 
• Inpatient ALF Facility Providing Primary Care / Wound Care
• Nurse Practitioner – Mohegan Lake, NY
• Minimum of two years clinical experience as an NP (preferably in a long term care setting).
• Preferably three days per week but open to two day.
• 12 – 15 hours per week
• W2 $60,000 base + upon reaching 150 billable visits per month, $35 for every additional visit OR 1099 based on a per visit fee is also an option.
