    Job Description

    The Advance Care Planning Social Worker Facilitator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

    The Advance Care Planning Social Worker Facilitator is responsible for providing information, education, guidance, and support to patients and families regarding Advance Directives. This role will act as resource to patients, family and clinical care teams to enhance patient experience and understanding of medical goals of care to align with patient wishes.

    ** Responsibilities:**

    • Provide information, guidance, support and referrals to patients and families regarding Advance Directives and in conjunction with the Advance Care Planning department.
    • Engage individuals in thinking about and planning for their goals in the context of a serious illness or life limiting condition.
    • Identify and clarify patient’s choices and preferences regarding life sustaining treatment.
    • Engage patients on an individual basis in multiple locations.
    • Share the value of Advanced Directive through conversations with patients and families.
    • Participate in community organization events to promote Advance Care Planning program and awareness on value of Advance Directive document completion.
    • Conduct one-on-one meetings with patients and family to consult and complete Advance Directives. To discuss with patients and families Goals of Care with life sustaining illness/disease.
    • Participate in Care Conferences.
    • Review existing documents for accuracy.
  • Qualifications

    Qualifications

    ** Required :**

    • Bachelor’s degree in Social Services
    • Clinical experience emphasis on end of life discussions and goals of care.
    • Strong knowledge of Advance Directives, purpose, implementation and value to patients, family and care team.

    ** Preferred:**

    • Master’s degree in Social Services
    • Direct patient care experience in a medical setting
    • Licensed Clinical Social Worker (LCSW)
