    Overview: We are a provider-led organization focused on delivering primary and post-acute care services to senior living communities and residents. Our innovative approach emphasizes prevention and care coordination to enhance the well-being of seniors across our extensive network of facilities.

    Summary: The facility-based Advanced Practice Provider (APP) plays a crucial role in delivering direct patient care services within the Post-Acute Care setting.

    Essential Duties and Responsibilities:

    Embrace our company's philosophy to provide care that promotes the maintenance or enhancement of each patient's clinical status and quality of life.

    Conduct Annual Wellness Visits and History and Physicals as requested by facilities.

    Perform clinical interventional and episodic direct care visits, including re-evaluations as necessary.

    Evaluate and treat residents for chronic, co-morbid illnesses, providing ongoing care management.

    Review medications, orders, and diagnostic tests, and respond to clinical abnormalities appropriately.

    Coordinate care with collaborating physicians and other disciplines such as Therapists, Specialists, and Social Workers.

    Maintain communication with families and attend family conferences when possible.

    Perform other duties as assigned.

    Qualifications:

    Valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of operations, with current board certification in your field.

    Minimum of a Bachelors and Masters Degree in Nursing from accredited Colleges or Universities.

    Experience in nursing required, with some experience as a Nurse Practitioner/Provider preferred.

    Strong knowledge of nursing and clinical Nurse Practitioner critical thinking skills applied to aged, chronically ill, and convalescent residents.

    Strong leadership, interpersonal, and problem-solving skills preferred.

    Excellent written and oral communication skills.

    Punctuality, organizational skills, and ability to function within a team.
