    After-Hours Care Center RN - Greenville, OH, US • Full-time (FT): 36 hours/week, 6:00 pm - 6:30 am, weekends and holidays included • Part-time (PT): 12-24 hours/week, 6:00 pm - 6:30 am, weekends and holidays included • PRN: Flexible hourly shifts, both day and night available Seeking RN for direct patient care at the Hospice Care Center. Responsibilities: Responsibilities of Position: Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs, including: • Inpatient Care Center. • SIA/Continuous Care. • Referrals and Admissions. • Chart Reviews. • Other duties as assigned. • Provide patient/family services according to established Plan of Care and signed physician orders. Consult patient’s physician and/or Medical Director when necessary and initiate appropriate telephone orders for changes in Plan of Care. • Coordinate IDG planning for assigned patients. • Participate in the IDG analysis of the patient, resulting in the development, implementation, and revision of the IDG Plan of Care. • Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers. • Facilitate continuity of care by coordinating the transfer/admission of home-care patients to an inpatient setting. • Facilitate continuity of care by coordinating the transfer/admission of inpatients to home-care settings, if applicable. • Explain hospice services and benefits in regards to Medicare, Medicaid, and Private insurance, and know when to use Palliative Care Interventions and ABNs. • Obtain and enter orders from Medical Directors and Physicians when deemed appropriate. • Order appropriate Durable Medical Equipment (DME) for the patient. • Perform competent initial nursing assessments. • Work evenings and weekends as needed to meet the needs of the patients and families and the agency's backup schedule. • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements. • Maintain clinical competency for assigned responsibilities and procedures. • Other duties as assigned. Qualifications: Education and Experience: • Graduation from an approved School of Nursing. • Current RN licensure in the State of Ohio. • Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing. • Hospice experience preferred. Special Skills and Requirements: • Shall have a valid driver’s license. • Excellent customer service skills. • Detail-oriented with excellent communication, documentation, and organizational skills. • Ability to follow instructions. • Ability to work independently without supervision. • Travel to other sites as needed. Travel may be in inclement weather. • Ability to lift 25 lbs of supplies or equipment. • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking. • Ability to assist with patient care activities such as ambulation, transfers, and positioning.
