    Who We Are: Partners In Care, an independent 501©3 nonprofit, is the largest and oldest provider of home health, palliative care, and hospice services in Central Oregon. Our dedicated team of over 200 employees and approximately 150 local volunteers serves patients and families across a 10,000-square-mile area, including Deschutes, Crook, and Jefferson counties.     Location: Central Oregon

    Hours: Full and part-time available; a combination of weekdays and weekend nights between 5 pm and 8 am.

    Travel: Local/regional travel by personal vehicle during the working day is centralized from the Bend office and extends throughout Central Oregon. Mileage reimbursement is provided.

    Compensation:

    Flat Shift Rate or Standby Pay of $15.

    Base Pay-$40 to $58.67 (DOE) When called to provide patient care.

    Differentials-$2 for Weekend Shift and $5 for the Night Shift.

    Sign-on Bonus: $4,000 paid after 90 days of employment.

    What You Get to Do:

    Assess patients and families to determine health needs.

    Coordinate patient care with RN Case Managers.

    Provide physical assessments, diagnosis, and treatment planning.

    Administer medications and comfort procedures.

    Educate patients and families on healthcare instructions.

    Communicate with physicians and the healthcare team.

    Required Skills & Qualifications:

    Valid Oregon RN License.

    Ability to pass a background test, drug screen, and TB test.

    Graduate of an accredited nursing school.

    One to two years of recent medical care experience in an institutional setting is preferred. Ideal candidates will possess 1-2 years of home health or hospice experience.

    Current automobile insurance is required. A valid Oregon driver’s license and reliable transportation.

    Valid CPR Certification.

    Strong communication and interpersonal skills.

    Ability to work independently and collaboratively within an interdisciplinary team.
