    Position: After Hours RN Location: Greenville, OH, US Available Role: - Full-time RN position - 36 hours per week - 3 12-hour shifts: 8:00 pm - 8:30 am - Every 4th weekend and holiday rotation Job Summary: The After-Hours RN will provide direct patient care outside of regular business hours. Responsibilities: Special Skills and Requirements: • Shall have a valid driver’s license • Excellent customer service skills • Detail-oriented with excellent communication, documentation, and organizational skills • Ability to follow instructions • Ability to work independently without supervision • Travel to other sites as needed. Travel may be in inclement weather • Ability to lift 25 lbs. of supplies or equipment • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking • Ability to assist with patient care activities such as ambulation, transfers, and positioning Responsibilities of Position: • Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs, including: • Routine Homecare Visits • End of Life /Continuous Care • Referrals and Admissions • Care Center Assistance • Chart Reviews • Other duties as assigned • Explain hospice services and benefits in regard to Medicare, Medicaid, and Private Insurances, and know when to use Palliative Care Interventions and ABNs. • Obtain and enter orders from Medical Directors and Physicians when deemed appropriate • Order appropriate Durable Medical Equipment (DME) for the patient • Perform competent initial nursing assessments • Facilitate continuity of care by coordinating admission of home-care patients to inpatient settings (hospital, nursing home), as needed • Work evenings and weekends as needed to meet the needs of the patients and families, and agency backup schedule • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements • Maintain clinical competency for assigned responsibilities and procedures • Other duties as assigned Qualifications: Education and Experience: • Graduation from an approved School of Nursing • Current RN licensure in the State of Ohio • Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing • Hospice experience preferred
