    All for Health, Health for All, Inc. (AFH) has provided health services to the medically underserved and indigent community for over 40 years. AFH is a nonprofit Federally Qualified Health Center and certified Patient-Centered Medical Home whose mission is to eliminate disparities in access to health care for the underserved multi-ethnic diverse population of California and Nevada states. AFH’s services encompass general family practice, pediatric care, mental health, and behavioral health services as well as adult day health care services. We have served over 21,000 patients last year with over 81,000 visits across a dozen clinic locations. Our comprehensive and integrated care delivery model is paramount to our success in ensuring continuity and coordination of care for all patients throughout their disease management. We are currently recruiting Primary Care Physicians, MDs, PAs, and NPs for our newly opened location in Laguna Niguel. This presents a rewarding and exceptional opportunity to proactively engage with patients and our care team. The Provider is responsible for the prevention, diagnosis, and treatment of patients within the scope of his/her field of training. She/he is expected to be familiar with the mission of ALL FOR HEALTH, HEALTH FOR ALL, and work collaboratively to achieve it. They must be cognizant of and comply with AFH policies and procedures as well as state and federal regulations. The provider will work with other staff as part of an interdisciplinary patient care team. Responsibilities: Essential Duties • Obtain health history and perform physical examinations • Diagnose and counsel patients on a plan for treatment • Prescribe medicine in accordance with California law, clinical practice guidelines, and standard of care • Order referrals for patients requiring services not provided at AFH • Review incoming reports (ie: lab, x-ray, EKG) sign off and follow up as needed • Comply with the infection control plan and encourage compliance throughout the office • Participate in peer review and quality improvement including Patient-Centered Medical Home meetings, provider meetings, and any other meeting requiring physician attendance • Participate in pre-visit planning during daily care team huddles • As an integral part of the care team, respects patients’ values, and preferences, and helps to ensure patients are a part of the care team • Perform on-call duties as assigned per policy guidelines • Accepts other duties as delegated Qualifications: • Licensed to practice in California and maintain license through obtaining CMEs • Current D.E.A. certificate • Current ACLS certification • Current CPR certification • Able to manage multiple priorities • Ability to speak, understand, write, and type English as well as have basic math and spelling skills • Manual dexterity Language Skills Bilingual skills are preferred, but not necessary for this position. If an employee speaks a language other than English, they may be subject to proficiency testing.
