POSITION TYPE: FULL-TIME & PART-TIME POSITIONS AVAILABLE POSITION TITLE: SERVICE COORDINATOR THE COMPANY: Alma Health Skilled Services is a premier provider of service coordination through the Pennsylvania State waiver program. As a provider to the CHC-MCO MLTSS program, our participants will experience the attention they deserve that maximizes their quality of life. * We are a leading agency in Service Coordination for quality and compliance scores * We are an NCQA accredited agency JOB SUMMARY: Coordinates, develops and monitors the assessment, care planning process of all aspects of the consumer needs. Serves as an advocate and liaison on behalf of consumers. DUTIES TO INCLUDE BUT NOT LIMITED TO: * Complete intake enrollment/assessments, reassessments, and monitoring * Monitor participant's health, safety and welfare * Provider members with all information related to receiving waiver services, including list of providers (for services), freedom of choice, rights, responsibilities and liabilities * Collaborate with the consumers to meet their needs & goals * Develop an Individual Service Plan (ISP) using Person Centered Planning strategies * Maintain confidentiality of consumers information. * Provide service in accordance with existing policies, procedures and regulations from the company. EDUCATIONAL REQUIREMENTS: Service Coordinators must, have a Bachelor's degree in social work, psychology, or other related fields and 1 year of LTSS or 3 years of Case Management or other Human Services work.
