    House of Hope, a nonprofit organization, is looking for a detailed-oriented and well-versed Alumni Case Manager Coordinator to join our team! As an Alumni Case Manager Coordinator your position is essential in helping clients regain independence within the community and take charge of their recovery journey.

    Experience, Knowledge, and Skills:

    · Minimum of an Associate ’s degree in social work or psychology.

    · Bachelor’s Degree preferred.

    · Two (2) years’ experience working in the field of addictions and or mental health.

    · Knowledge of the recovery process and the ability to facilitate recovery.

    · Knowledge and skills to teach and engage in basic problem-solving strategies.

    · Ability to work with a diverse population.

    · Effective communication skills.

    · Must be an independent thinker.

    · Computer Literate

    Duties and Responsibilities:

    · Assess clients' needs and link to appropriate resources.

    · Develop and facilitate engaging group sessions that focus on relapse prevention strategies and essential life skills.

    Collaborate with clients to develop individualized recovery plans, goals, and strategies for success.

    Assist clients in obtaining meaningful vocational and educational experiences.

    Network with agency and outside service providers, ensuring client access to appropriate resources.

    Additional task and duties as assigned.
