Now Hiring: Certified Nursing Assistant (CNA) for Adult Hospice and Home Health
We are seeking a dedicated and compassionate Certified Nursing Aide (CNA) to provide essential healthcare services to patients in their homes. The CNA will play a vital role in delivering care between visits from primary healthcare providers, assisting individuals with disabilities, chronic conditions, and those recovering from surgery. This role also involves providing personal support and aiding in the rehabilitation process for patients in need of ongoing care.
Essential Duties and Responsibilities: The core responsibilities of this position include, but are not limited to:
Patient Care: Provide health services in the home environment, including assistance with activities of daily living.
Medication Administration: Administer prescribed medications as directed, ensuring safe and accurate delivery.
Personal Care Support: Assist patients with personal care, including bathing, dressing, grooming, and other daily hygiene tasks.
Vital Signs Monitoring: Monitor and record vital signs (temperature, respiration, pulse, blood pressure), and report significant changes in patient conditions.
Documentation: Maintain accurate and up-to-date records of patient care provided and document any observations or changes in the patient's condition.
Physical Assistance: Aid patients with mobility, physical exercises, and therapy routines as directed by healthcare providers.
Patient and Family Support: Educate patients and families on proper nutrition, exercise, and lifestyle choices that enhance the patient's recovery and well-being.
Emotional Support: Provide companionship and basic emotional support to patients, ensuring their comfort and mental well-being.
Minimum Qualifications (Knowledge, Skills, and Abilities):
Certification: Current Nursing Aide Certification.
CPR Certification
Valid Drivers License
Home health care experience preferred.
Basic computer skills required; light typing preferred.
Strong ability to communicate effectively with patients, families, and healthcare teams. Bilingual (Spanish) preferred.
Ability to work independently, manage time effectively, and prioritize tasks in a dynamic care environment.
Reliable transportation and auto liability insurance are required for travel to patient homes.
Education:
High School Diploma preferred. Has successfully completed one of the following:
A) Training program/competency eval as specified in 418.76 (b)(c) of the Medicare regulations
B) Competency eval program that meets 418.76 (c)
C) Nurse Aide training/competency eval program approved by the state and listed in good standing on the state nurse aide registry
D) State Licensure program that meets requirements of 418.76 (b)(c)
Experience: At least one year (preferred) as a hospice aide or nursing assistant in a hospital, nursing home, home health agency, hospice, or long term care facility or completion of Certified Nursing Assistant or Certified Home Health Aide Program.
