    Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs.     Position Summary:
    We have an exciting opportunity to join our team as a Assistant, Patient Navigator.

    In this role, the successful candidate We have an exciting opportunity to join our team as an Assistant Patient Navigator The Navigator will play a critical role in our Enhanced Treatment and Recovery EnTRy program a program designed to improve early detection and treatment of mental illness particularly psychotic spectrum disorders across the lifespan The Navigator coordinates patient care services as well as provides outreach education and referral services to patients The Navigator is responsible for carrying out tasks that are needed to execute the medical and support service plan Depending on grant requirements and population served the Navigator may provide rapid HIV testing and pre and post test counseling

    Job Responsibilities:

    Ensures patients make and keep appropriate health and social services appointments; accompanies patients to primary care appointments and to other health care and social service encounters as appropriate


    Provides education risk reduction counseling and skill building services to the patients as necessary per patient needs and grant requirements

    Depending on grant requirements and population served may provide rapid HIV testing and pre and post test counseling

    Performs entitlement reassessment and follow ups

    Coordinates ongoing navigation and logistical support for appointment keeping reminders transportation and childcare arrangements as appropriate

    Provides critical information to other members of the health care team based on observations in the field or in patient encounters

    Educates coaches and empowers patients

    Depending on needs may provide DOT Daily Observed Therapy ; if providing DOT must observe and record patient self administration of medications

    Participates in trainings conferences meetings and quality improvement efforts as assigned

    Minimum Qualifications:

    NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $33,778.63 - $41,891.20 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
