We are seeking a dedicated and empathetic Associate Care Manager to join our team. This role involves coordinating and managing the care plans for our geriatric clients, ensuring they receive the highest standard of care.

POSITION SUMMARY: Under the full direction of the Geriatric Care Manager, the Associate Care Manager provides individual and family assessments on the overall health & well-being of an individual. This position makes recommendations for programs, provides referrals and oversight of the client and provides follow-up on medical appointments to the family and those involved in the individual’s care. The goal of this position is to gain the adequate knowledge and skills required to confidently advance into the Geriatric Care Manager role. This position requires a minimum of one year experience in care management, social work, nursing or a similar field with a minimum of an Associate degree.
CLIENT ENGAGEMENT:
Conducts individual, couple and family assessment and offers timely, practical alternatives. Takes thorough and accurate notes at client appointments and asks provider adequate questions in order to obtain the most factual and beneficial information for the client. Provides referral, monitoring and oversight of implemented care as well as recommending revisions and providing support and education. Provides timely and thorough updates to client circle of influence as desired by client. Provides support, education and advocacy to client as well as formal and informal supports. Leads clients to development of strategies for conflict resolution. Reviews and evaluates client progress and establishes follow-up contact. Interfaces with other healthcare providers in order to achieve success in managing client needs. Maintains confidential client records. Assists in development of community based, referral network. Organizes, coordinates, and conducts reviews of community resources and social service agencies and other psychosocial referral sources for clients. Plans and implements outreach activities and promotional campaigns. Manages delivery of services to targeted program participants or beneficiaries. Participates in peer counseling in order to achieve the best possible client outcomes. Accompanies clients to medical provider appointments and orchestrates follow up regarding physician orders/recommendations. Maintains proper documentation of correspondence, medical visits, and care management documentation within 24 hours of provision of service.
POSITION QUALIFICATIONS:
Minimum Education: Associate Degree
Preferred Education: MSW, BSW, RN
Preferred Experience: 1 Years
Preferred Field of Expertise: Geriatric Care Management, Social Work, Nursing
Preferred Certification: Must be eligible for a path to care management certification once adequate experience is achieved based upon meeting experience/supervision requirements.
POSITION REQUIREMENTS:
Must have/maintain Automobile Insurance Coverage 100/300/100 and list Geriatric Resources, LLC as an Additional Interest on the policy.
Have a clear National Criminal Background Check upon hire and annually.
Must remain certified in CPR & First Aid.
Must be eligible for work in Arizona through E-Verify screening.

COMPENSATION:
$28.36-$30.00/Hour
