    The RN Baylor position provides support to fulltime RN case manager’s throughout the team to manage overflow in visits one day per week. This position also works Saturdays and Sundays performing Admissions, emergent and routine visits as needed. The RN Baylor is responsible for oversight and coordination of the patient hospice plan of care during their visits and coordinating back with IDG team, and/or RN case manager any changes in patient plan of care or needs. This positions overall responsibility is to provide direct patient care in the community setting on weekends in accordance with physician’s orders. This is a full-time role, working one day per week and the weekends.

    Job Responsibilities:

    Regularly assesses and reassesses the nursing needs of the client. Competently implements 24-hour responsibility for each client including nursing and home health aide service.

    Provides direct client care and case management as needed. Performs admission procedure as per the policies using accurate documentation. Develops a plan of care, in conjunction with the IDG team and patient/family, consistent with the physician’s plan of treatment.

    Coordinates the implementation of the plan of care for clients residing in SNF, ALF, hospital contract bed or personal residence.

    Initiates and provides nursing services, treatments and preventive procedures. Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders.

    Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the client’s physical or emotional condition.

    Participates in rotating on-call system and is responsible for providing on-call coverage on an as needed basis.

    Qualifications & Experience:

    Graduation from an accredited school of nursing; BSN preferred

    Currently licensed to practice as a Registered Nurse in the State of Connecticut.

    A minimum of two years of nursing experience. Home Health Care and/or Hospice experience preferred. Active client contact within past three years, preferred.

    Is computer literate, enters information into computer in a timely and accurate manner. Uses electronic means of communication effectively within agency policies and practices.

    Ability to supervise others appropriately.

    Ability to respond appropriately to crisis outside of a hospital setting.
