    Legacy Oaks of Sacramento Assisted Living and Memory Care is seeking an experienced Behavior Support Coordinator to join our compassionate team. If you are dedicated to improving the quality of life for residents through behavior management and fall prevention programs, we’d love to meet you!

    Key Responsibilities:

    • Program Leadership : Lead initiatives like the Turning Point Initiative and Falling Star Program to prevent falls and manage behaviors.
    • Resident Assessment : Conduct behavioral and fall risk assessments, creating personalized care plans to address physical, emotional, and behavioral needs.
    • Team Collaboration : Work closely with interdisciplinary teams, including Resident Support Specialists, Medication Room Coordinators, and more.
    • Staff Training : Train staff on de-escalation techniques, fall prevention strategies, and behavior management.
    • Family & Resident Engagement: Serve as the primary contact for families, providing updates and conducting education sessions on care strategies.
    • Crisis Management : Respond to emergencies, implementing de-escalation protocols and coordinating with external healthcare providers.

    Qualifications:

    • Education : Bachelor’s degree in psychology, social work, healthcare management, or a related field (or equivalent experience).
    • Experience : Minimum 5 years in elder care, behavioral health, or fall prevention programs.
    • Skills : Strong leadership, advanced knowledge of behavior management strategies, and expertise in fall prevention protocols.
    • Licensure & Certifications: TB certificate, First Aid & CPR certification, and criminal background clearance.
    • Additional : Experience working with the elderly population and familiarity with Title 22 and DHCS guidelines preferred.
